Inadvertent administration of 100% oxygen during anaesthesia.

چکیده
Sir,—Modern anaesthetic machines have a device (usually referred to as "the oxygen bypass" or "emergency oxygen") which enables the anaesthetist to administer high flows of 100% oxygen. When a patient is connected to an anaesthetic machine it is possible to overlook the fact that the emergency oxygen device is in the "on" position and is delivering 100% oxygen. This occurrence is most likely if the device operates quietly and uses a "push-pull" switch. The Cape Waine Mark II anaesthetic machine and ventilator has such a mechanism. Accidents have occurred in our operating theatre on two occasions when patients had been transferred from the anaesthetic room to the operating theatre and connected to a machine. On both occasions a junior anaesthetist failed to notice for some time that the emergency oxygen device was "on" and the paralysed patients received higher concentrations of oxygen than intended. The following day, not unexpectedlyj the patients complained of awareness during anaesthesia.

Inadvertent inhalation anaesthesia during surgery under retrobulbar eye block.
I describe a case of inadvertent inhalation anaesthesia during surgery under retrobulbar anaesthesia and its management. Some of the hazards of supplementary oxygen delivery during monitored anaesthetic care and the actions taken to prevent this mishap recurring are discussed.
متن کاملAnaesthesia for a patient undergoing transthoracic endoscopic vagotomy.
We describe the anaesthetic management of a patient who underwent transthoracic endoscopic vagotomy. One-lung ventilation was necessary to provide adequate surgical access. Potential intraoperative problems involved arterial oxygen saturation during one-lung ventilation, unequal intrathoracic pressures causing mediastinal displacement and inadvertent myocardial injury by surgical instruments an...

Objective: This prospective study performed to evaluate blood biomarkers alterations with administration of propofol for maintenance of anaesthesia during long oral and maxillofacial surgeries in order to estimate the risk of Propofol Infusion Syndrome (PRIS). This rare syndrome often would be happened in long duration or high dose infusion which is characterized by the combination of metabol...
متن کاملIncreased oxygen administration during awake carotid surgery can reverse neurological deficit following carotid cross-clamping.
We describe the management of two patients undergoing awake carotid surgery who developed signs of cerebral ischaemia following cross-clamping of the internal carotid artery. Administration of oxygen 100% with a close-fitting anaesthetic facemask reversed the neurological deficit, avoiding the need for insertion of an internal carotid artery shunt. Thus, the incidence of shunt insertion, which ...
متن کاملSedation during spinal anaesthesia: a case for the routine administration of oxygen.
We have studied the effect of sedation with midazolam on arterial oxygen saturation during spinal anaesthesia in two groups of patients: one group received supplementary oxygen, the other group breathed room air. A significant reduction in oxygen saturation was observed in patients not receiving supplementary oxygen; six of 15 patients in this group developed hypoxaemia or severe hypoxaemia whi...
